PATIENT MANAGEMENT,  ELECTRONIC BILLING AND SOAP NOTE SOFTWARE 

Blue Cross and Blue Shield of Michigan will require you to report your NPI's to them along with all of your legacy ids by filling out their "Professional Provider Authorization" form--online.

First:

1. Click on the [Customize] menu
2. Click [Providers]
3. Click [Print]
4. Click [Ok] You will now have a printed list of your legacy provider id's.

Second:

Get your assigned 5 character BCBSM billing location code:

1. Click on the [Customize] menu
2. Click [Insurance Companies]
3. Call up Blue Cross--usually BC001
4. Click on the [Electronic Billing] tab page
5. Write down what you see in the "Login" field
    (eg: c0???) - this is your billing location code

Third:

Fill out the BCBSM "Professional Provider Authorization" form

1. Type in your "User ID" and "Password" then click enter
    (if you do not know your user id or password--contact BCBSM at 248-486-2292)
2. Click on "Professional Provider Authorization"
3. Simply fill out the form online.
    Be sure to enter your billing location code (eg: c0???) in the "Submitter ID" and "Unique Receiver ID"
